I suspect it was one of those incidents where it took 5 thing to all coincide. The railway is not heavily used, the crews all should have been aware of what others were doing and where.

But then, I think that the fully loaded train has a manned caboose on that line, so you'd think there would have been some sort of alert?

This is so awful, have been listening to the news trickle in all day and was hoping for a somewhat positive outcome. Feel bad for all involved.

This might be a record time to wrap up an investigation.
An investigation into a Vancouver Island train derailment that killed three people and left two others seriously injured won’t result in criminal charges.
Mounties announced Monday the investigation is being turned over to the Transportation Safety Board and WorkSafe BC.
